Privacy Note

Newman Knights Hockey Club Inc. (NKHC) is bound by the provisions of the Commonwealth Privacy Act. NKHC collects your personal information for the purposes of informing you of club activities, to contact you in the event of an emergency and to provide to HockeyWA for registration details and sponsors of the club for fundraising purposes.

Sponsors may use this information to contact you to make special offers which are available to club members. Should you NOT wish for NKHC to provide your information to sponsors please notify us by placing a cross in the box below. You have the right to access your personal information held by NKHC subject to exceptions contained in the Privacy Act.

Hockey WA and HockeyNet

“In registering with Newman Knights Hockey Club you acknowledge that your information will be passed onto Hockey WA as the peak governing body for the sport of hockey in Western Australia. Hockey WA will use this information to contact you with its Bulletin newsletter and other relevant notifications. The use of your information is strictly governed by the Hockey WA Privacy Policy which is available here you may opt out at any time by following the instructions emailed to you when Hockey WA receives your information or by emailing admin@hockeywa.org.au. Hockey WA will not sell, rent, or otherwise give your information to a third-party without your consent.”

UMPIRING: - As a player, I understand that I am to be available for umpiring duties as required by the Club. Umpiring duties generally coincide with team byes during the year and the Club’s umpiring co-ordinator allocates appointments to umpire. Some club members may undertake umpiring duties on your behalf for a fee – see the umpiring co-ordinator for details. Non-attendance attracts large fines from Hockey WA ($100 per game) and I understand this cost is passed on to the nominated players/team who don’t turn up for umpiring duties or who fail to find a suitable replacement. All players from all grades are expected to participate in the State Officiating Program (SOP 1) to further enhance umpiring skills and confidence.

INSURANCE AND INDEMNITY: - I understand that hockey can be a dangerous sport and the risk of injury whilst playing or training is high. I agree that it is my responsibility to arrange and take medical or other insurance necessary to cover any such injury to myself and that the Newman Knights Hockey Club will not necessarily have any insurance on behalf of its members to cover such claims. I agree to indemnify the Club, its Committee, coaches or other authorised representatives against any claims for injury, expenses or damages suffered by me, except as a result of the wilful negligence of that party. I also agree to play in a safe manner according to the rules of the game and to follow instructions from coaches, captains and officials. If I breach these conditions, I understand I may be liable to sanctions imposed by the Club and Hockey WA.

FUNDRAISING: - In order to keep down the cost of playing fees and to pay for equipment the Club runs a variety of fundraising activities. As a Club Member you will be expected to participate in as many fund raising activities as possible.

CLUB FUNCTIONS: - Our committee works hard to organize Club functions. As a Club Member you will be expected to support the Club by attending as many functions as possible throughout the year.

KNIGHT MAIL: - The Club regularly contacts its members to advise them of important information, articles and functions during the season via email, please log on to the club website www.newmanknights.com.au to ensure your contact details are up to date.

CONDUCT: - As a Club Member you will be expected to conduct yourself at all times in a fair and sportsman like manner, bringing credit to our Club. Failure to do so can result in your membership being withdrawn. The club’s code of conduct can be found on the website.

HOCKEY WA & HOCKEYNET: - For the 2011 season Hockey WA will require clubs to enter player email and postal addresses into HockeyNet. This initiative is aimed at circumventing the shortcomings of using the club contact method of communicating with players and to address the notification requirements associated with ASIC Class order CO 08/1 (which affects Hockey WA as a Group Purchasing Body of a Risk Management Product).

It is not Hockey WA’s intention to on sell this database to third parties or spam players. This database will merely be used for such purposes as to keeping players up to date with Hockey WA activities, distribute important notices and distribute information on HWA sponsorship opportunities. Players have the option to opt out at any time.
